Selecting the right approach — or combination — depends on individual disease stage, symptom pattern, prior treatment history, and clinical assessment.

When surgery becomes the realistic next step

Injections work well for many people with post-meniscectomy knee OA, but they are not indefinitely effective for all. When symptom control has deteriorated and has not been adequately re-established within roughly 6–12 months of optimal injection management, a surgical review is a reasonable next step — not a sign that treatment has failed, but a signal that the pathway needs to move forward.

The spectrum of surgical options is wider than many patients expect. For younger patients with contained, focal cartilage defects, cartilage repair or restoration procedures may be appropriate — addressing the structural source of symptoms rather than replacing the joint. Someone developing end-stage knee OA after a meniscectomy in their thirties may be seeking surgical input in their late forties or early fifties — potentially 15–20 years younger than the typical knee replacement candidate. Published data indicate that revision rates after knee replacement rise meaningfully when the procedure is performed at a younger age, since prosthesis longevity is finite. Every year gained through joint-preservation — osteotomy, cartilage repair, or sustained injection management — reduces the likelihood of needing a second operation.

The specific question worth pressing in any surgical consultation at this stage is whether joint-preservation has been explicitly considered and, if not recommended, why — rather than moving directly to a replacement discussion.
