
A different joint, a different disease
Most people who receive a patellofemoral osteoarthritis diagnosis have already been told they have 'knee arthritis' — as though every form of knee degeneration is the same condition with the same trajectory. It is not.
Patellofemoral OA (PFOA) affects the joint between the underside of the kneecap and the trochlear groove at the front of the femur. Tibiofemoral OA, the more commonly recognised form, affects the main weight-bearing compartment lower down, where the femur meets the tibia. These two compartments are anatomically separate, and they can degenerate independently — isolated PFOA is a recognised clinical entity, not a diagnostic shortcut for patients with poorly localised knee pain.
Crucially, PFOA is not the final chapter of generalised knee degeneration; it can be the opening one. Research by Stefanik (2016) and Musumeci (2017) found that structural OA changes can begin in the patellofemoral compartment before the tibiofemoral joint shows any involvement — meaning PFOA can exist, progress, and cause significant disability while the rest of the knee remains largely intact.
That distinction is not merely academic. It shapes which imaging views are needed to see the damage, which clinical signs point to the right compartment, and which treatments are most likely to help.
What PFOA actually feels like
The pain tends to arrive at specific moments: going down a flight of stairs, pushing up from a low chair, squatting to pick something up, or kneeling on a hard floor. These are the movements that press the kneecap firmly against the trochlear groove, loading the damaged cartilage surfaces — and it is this activity-specific pattern that gives PFOA its characteristic shape.
Walking on flat ground, by contrast, is often well-tolerated, at least in earlier stages. This is one of the clearest practical distinctions between PFOA and tibiofemoral OA: tibiofemoral disease tends to make sustained walking increasingly painful; PFOA can allow someone to cover ground comfortably while still struggling badly on a single flight of stairs.
Episodes of significant swelling are commonly reported, along with a sense of weakness or giving way — not the mechanical locking associated with meniscal injury, but a feeling that the knee may buckle under load. A 2012 population-based study by Peat and colleagues (745 adults with knee pain) identified markedly reduced quadriceps strength as a specific clinical feature of moderate-to-severe isolated PFOA, separating it from the generalised weakness seen across knee conditions.
Symptoms are often intermittent, particularly early on. Patients frequently describe long gaps between flares, which can delay recognition that anything structural is developing — and can make the condition easy to dismiss before it is properly assessed.

Who is at risk and why
Three distinct clusters of risk feed into PFOA, each through a different biological mechanism.
How the kneecap tracks within its groove is the most fundamental. When the patella is dysplastic — poorly shaped for the trochlear groove — or chronically maltracked, the load it generates concentrates on a smaller cartilage area rather than spreading across the full contact surface. Sustained over years, that asymmetric pressure accelerates wear. Patellar alignment and joint congruity on radiograph are now established as independent predictors. A 2023 MRI-based shape study adds an earlier signal still: a relatively thicker dorsal bump on the articular patellar surface is associated with structural OA abnormalities even in adults under 40, suggesting patellar geometry may be identifiable as a risk factor well before symptoms develop.
Injury history introduces two less obvious pathways. After ACL injury, PFOA prevalence in published series ranges from 4.5% to 80%, depending on follow-up length, graft choice, and surgical timing — bone-patellar tendon-bone grafts, single-bundle reconstruction, and delayed surgery all feature as contributing factors. This is clinically relevant: post-ACL patients who develop anterior knee pain should raise it with their clinician rather than attribute it to general post-surgical sensitivity. Separately, lateral meniscal tear and extrusion carry measurable consequences for the lateral patellofemoral compartment: data from the Multicenter Osteoarthritis Study (MOST cohort) show approximately a 1.7-fold increased risk of lateral patellofemoral cartilage worsening over two years.
Higher BMI increases patellofemoral joint loading with each step-down or squat, and functions as an independent risk factor across these structural and injury-related pathways.
How PFOA is assessed
A specialist assessment for suspected PFOA differs in meaningful ways from a routine knee examination — and those differences help explain why some patients receive unhelpful reassurance for years before an accurate picture emerges.
On examination, the consultant will specifically test patellofemoral compression — pressing the kneecap against the trochlear groove while the knee is extended — and assess quadriceps strength. These are not standard components of a general knee check. Peat 2012 confirmed that moderate-to-severe isolated PFOA is characterised by pain on compression, valgus deformity, and a conspicuous absence of the signs that dominate tibiofemoral disease: no varus deformity, no effusion, no mediolateral instability, no meaningful reduction in flexion range, and no bony enlargement.
Imaging often surprises patients who have already had a knee X-ray and been told it was normal. Standard anteroposterior views, taken weight-bearing, show the tibiofemoral joint clearly; the patellofemoral compartment barely registers on them. Skyline (axial) views — taken with the knee flexed — are required to reveal any narrowing between the underside of the patella and the trochlear groove.
MRI adds further detail when surgical planning or injection decisions are under consideration: it can quantify cartilage integrity, identify bone marrow lesions, and assess patellar alignment parameters in ways plain X-ray cannot.
A confident diagnosis requires all three elements — symptoms, clinical signs, and imaging — to align. An isolated imaging abnormality without a corresponding clinical picture does not constitute a diagnosis on its own; the findings must be read together before a management plan is appropriate.
Treatment options from first-line to surgical
Conservative management is the appropriate starting point for most patients, and the evidence supports a more optimistic prognosis than many people expect. With a structured physiotherapy programme, some 67% of patients achieve meaningful symptom resolution within six months; over a seven-year horizon, around 80% rate their knee as excellent. Activity modification — stepping back from deep squatting, lunging, and heavy stair-climbing, and substituting walking or swimming — reduces peak patellofemoral load without requiring complete rest.
Among the conservative measures, quadriceps rehabilitation deserves specific emphasis rather than being treated as generic knee strengthening. Because patellofemoral loading is directly governed by how the quadriceps absorbs and distributes force through the joint, building this muscle group is a therapeutic lever specific to PFOA — the link established in the Peat 2012 assessment data, where markedly reduced quadriceps strength was one of the defining features of moderate-to-severe isolated disease.
Injection options
When conservative measures provide insufficient relief, injectable therapies offer an additional step before surgery is considered. Platelet-rich plasma has produced clinically meaningful improvements in published series: a 2025 systematic review of five studies (146 patients) reported VAS pain scores falling from 6.7 to 2.1 and Kujala function scores rising from 71 to 83. These are encouraging signals, but radiological evidence of cartilage regeneration remains limited and the review authors call for well-designed sham-controlled trials before firm conclusions are drawn. Polyacrylamide hydrogel showed a reduction in patellofemoral bone marrow lesions following a single injection in a 2022 pilot study of advanced PFOA — an early finding that warrants further investigation rather than extrapolation.
Surgical pathways
For isolated end-stage PFOA that has not responded to the above, patellofemoral arthroplasty — replacing the patellofemoral compartment alone — achieves good functional outcomes with a revision rate of just 3.9% in published series. Crucially, revisions are driven predominantly by progression of tibiofemoral OA into the adjacent compartment, not by failure of the implant itself; patients can reasonably be reassured that the procedure is not inherently fragile, but that the underlying disease process in the knee requires monitoring.
When PFOA occurs alongside tibiofemoral disease severe enough to warrant total knee replacement, the question of what to do with the patella becomes clinically significant. Evidence from a direct comparison of 30 patients shows patellar resurfacing produces significantly better range of movement, stair-climbing ability, and pain relief than patelloplasty within the TKR setting.
Prognosis and when to seek specialist input
For most people who receive this diagnosis, the trajectory is more manageable than the label 'osteoarthritis' might suggest. Structured physiotherapy produces durable improvement in the large majority of patients — which means committing to conservative care is well-supported rather than a concession to doing nothing.
Certain features should prompt a specialist assessment rather than continued self-management:
- Anterior knee pain that has not meaningfully improved after six to eight weeks of structured physiotherapy
- Episodes of significant swelling
- Functional limitation affecting stairs, work, or daily mobility
- A background history of ACL or meniscal injury, which may have altered patellofemoral loading patterns in ways that influence both prognosis and rehabilitation design
Monitoring matters over time. PFOA can advance into the tibiofemoral compartment, a risk that is higher in younger patients or those with structural alignment abnormalities — making periodic review worthwhile even during stable phases.
Research into sex-based differences in patellofemoral loading is ongoing; preliminary work suggests gait patterns may warrant individual tailoring in rehabilitation, though no confirmed RCT guidance exists yet.
Any escalation — whether to injection therapy, cartilage intervention, or surgery — should follow a specialist assessment that correlates current symptoms with up-to-date imaging. Imaging findings alone are not sufficient to guide that decision.
